Price List 2012-2013

Your Student Health Center Fee covers most medical services at the Engemann Student Health Center however, there are some additional fees described below. If you have any questions regarding cost and services, please contact Cashier Services.

Appointments

Medical Services Appointments

• Visit: Covered by Health Center Fee.
• Late appointment cancellations or no shows: $20. A two-hour advance notice is required to cancel an appointment. Call to Cancel an Appointment.

Counseling Services Appointments

• Individual Session: Covered by Health Center Fee. 12 session limit.
• Group Session: Covered by Health Center Fee.
• Late appointment cancellations or no shows: $20. A 24 hour notice is required. If you miss two intake appointments (either by canceling or not showing), you will be referred to the Director of Counseling Services for assistance.

Allergy Clinic

• Allergy Consultation: Covered by Health Center Fee, referral by doctor for new patients
• Allergy Desensitization – Patch: $144, by referral or doctor order
• Allergy Desensitization – Skin Testing: $100, by referral or doctor order
• Allergy Desensitization – 1 Injection: $7, by referral or doctor order
• Allergy Desensitization – 2 Injections: $12, by referral or doctor order
• Allergy Desensitization – 3+ Injections: $18, by referral or doctor order

Health Information Management

• Photocopying of medical records: 5 pages or less: no charge, 6 pages or more: $10. Authorization For Disclosure of Medical Information form required
• Retrieval From Storage: $10

Immunization Clinic

• Gardasil: $162 per injection. Series of 3.
• Hepatitis A: $59 per injection. Series of 2.
• Hepatitis B: $59 per injection. Series of 3.
• Influenza (Flu): $25
• Measles/Mumps/Rubella (MMR): $68. Part of New Student Health Requirements
• Menactra (Meninggococcal): $137
