Your medical records are a confidential record that is created when you visit any USC Student Health medical services provider, or Office for Wellness and Health Promotion staff member for an individual consultation. If you visit Counseling Services, you will also have a medical record on file in that location.
No one is authorized to see your medical record without your written authorization if you are over 18 years of age, according to the Confidentiality of Medical Information Act (CMIA). Exceptions to this include: medical emergencies, utilization review, quality assurance activities, court order or subpoena, and licensing and accreditation of the facility.
